I declare that all the above particulars are correct. If I am submitting on behalf of the employer, I declare that I am authorised by the employer to provide the information.

I have read and understood Inter Great Employment Pte Ltd's PDPA policy, and consent to Inter Great Employment Pte Ltd collecting, using, disclosing or otherwise processing my family's personal data disclosed here in accordance with the Personal Data Protection Act ("PDPA").

I consent to the Singapore government and statutory authorities obtaining and verifying my information, including medical conditions, with any person, organisation or any other source, and using the information to assess my eligibility as an employer.

I consent to the Ministry of Manpower displaying my/our particulars on the application form.
